Description:We have a Dell 1950 Server with PCI-X interface and one TE412P Digium Card. We are running Asterisk 1.2 for a long time but we decided to go to 1.4.21.2 and zaptel 1.4.12. After the upgrade and with no specific reason we get Linux Crashing. Dell reports PCIX Error on PCI Buss at which the Digium card is connected. We tried different cards as well and we get the same error. When we downgrade to 1.2 driver everyting works fine. We also tried Dell 1950 III which has the same issue. We believe that the problem is on zaptel driver but we can not get any further info because the system is unusable after the crash.

Please contact Digium technical support for help with this issue.  Based on what you've written, I believe the driver update is exposing an issue with older versions of the firmware on the TE412P and your host system, and a firmware update most likely will resolve your issue.
