Description:Blind transfers hang up. 100% reproducible on my system.  Not a problem on 1.6.2.13, but the problem exists on 1.6.2.15-rc1.

****** STEPS TO REPRODUCE ******

An incoming DAHDI call to a Polycom phone.  Press Transfer, then blind, then any extension that transfers to a SIP phone (including a blind transfer to the exact same SIP account).  Call will hangup.

The exact same steps will work fine on 1.6.2.13.

****** ADDITIONAL INFORMATION ******

Console output, verbose 3.  In this case calling extension 200, blind transferring to extension 200 again.  But any blind transfer will fail.

== Using SIP RTP CoS mark 5
   -- Called SIPACCOUNT__A
   -- SIP/SIPACCOUNT_A-00000019 is ringing
   -- SIP/SIPACCOUNT_A-00000019 answered DAHDI/6-1
   -- Started music on hold, class 'default', on DAHDI/6-1
   -- Stopped music on hold on DAHDI/6-1
 == Spawn extension (internal-transfer, 200, 1) exited non-zero on 'DAHDI/6-1'
   -- Hungup 'DAHDI/6-1'

This appears to be a duplicate of issue ASTERISK-16847.  Please try the SVN 1.6.2 branch to see if this resolves your issue.

SVN works fine, thank you.
