The problem can't be reproduced, it's random.

Asterisk version is 1.6.0.14

You're using a pretty old version of 1.6.0.  It is very likely that this is fixed in a later release.  However, if it's not, you will need to upgrade to 1.6.2 before we can look at it.

Per the Asterisk maintenance timeline page at http://www.asterisk.org/asterisk-versions maintenance (bug) support for the 1.6.0 and 1.6.1 branches has ended. For continued maintenance support please move to the 1.6.2 branch.
