Description:extensions.conf.sample says:

 174 ; Contexts contain several lines, one for each step of each
 175 ; extension, which can take one of two forms as listed below,
 176 ; with the first form being preferred.  One may include another

but there is only one such format:

 182 ;exten => someexten,{priority|label{+|-}offset}[(alias)],application(arg1,arg2,...)

The second one, which was:

;exten => someexten,{priority|label{+|-}offset}[(alias)],application,arg1|arg2...

has been deleted in the 1.4 to 1.6 transition.

Remove "second form" of extensions, as it no longer applies.  Also, cleanup
the grammar, formatting, and introduce several clarifications to the text.
