Please try the following branch and see if it resolves your problems:

svn co http://svn.digium.com/svn/asterisk/team/file/audiohooks-1.4

Will do. What's the difference between the branches?

It includes a backport of the chanspy/mixmonitor architecture re-write that is already in Asterisk trunk.

Happened again twice. Can I revert back to the regular branch? Perhaps I should go back to 1.2?

The situation seem MUCH worse with audiohooks - just got a crash from mixmonitor (bt6). This is  production system - going back to 1.4.16.2 for now.

Would it be possible to get access to the machine where the audiohooks branch caused these crashes so I could examine the backtrace closer?

Fixed in 1.4 as of revision 98975.
