Attempt to add rules via 'Add A Calling Rule' button seem to work, but do NOT appear in the calling Rules list.
ie: Have NO Calling Rules now and cannot add any via GUI

Calling Screen Displays Following Text with buttons, but NO Calling rules anymore??!!

What browser are you using? I'm not able to duplicate your issue

Problem occurred using Opera 9.10 build 8679 Win 32 Windows XP as was not able to log in using Firefox.  Have reset / cleared history in both FireFox and can now use again for login.
Am not able to add any calling rules now, using FireFox 1.5.0.8 on WinXP, FireFox 2.0.0.1 on Win2003Svr nor Opera.
Is there a way to reset the GUI to the default Dial Plan ?

At the terminal login as 'admin' with your password. 'sudo su -' to get root level access. Backup any configs you want to keep in /etc/asterisk. Default asterisk configs are in /usr/share/asterisk/configs. You can copy the ones you want out of there and into /etc/asterisk. Then you'll want to add a user to manager.conf, append the following to the default manager.conf file and copy that into /etc/asterisk/

[admin]
secret = mysecret
read = system,call,log,verbose,command,agent,user,config
write = system,call,log,verbose,command,agent,user,config

and to make sure your new changes get loaded up correctly, restart asterisk

# service restart asterisk

Using your browser try logging into the gui with 'admin' / 'mysecret'
