What about realtime?  Seems silly not to integrate this directly into the main configuration file.

Hmm.. Can You explain what do You mean? As I know, app_voicemail already supports realtime. This patch is workaround for current situation with users.conf. And probably replacement for passwords, stored into voicemail.conf directly.
My thought was, that it's better to store passwords in one place, instead of maintaining two configs: voicemail.conf and users.conf (issue 8436)
If this is suitable, some code can be removed from app_voicemail.

This feature needs to wait for the proper resolution of 8436 before any discussion may proceed.  I am suspending this feature until then.
