Description:This is to continue in the idea of the bug ASTERISK-2722755.
With that patch, it lets you the possibility to choose the name of the SQL table where insert will occur.

I've added the possibility to unload the module (if you need to).

if someone can test this out, it would be appreciated.

More people can test, higher are the probablities to see this merging into CVS.
