To reproduce this, add a call to a queue, accept the call from the queue and then transfer it to another extension. For the duration of that first call, any new calls coming into the queue will not be routed to that agent. show agents will indicate the first call still terminated with that agent.

You should be using # transfers. You cannot use the transfer button with agents as described in an earlier bug report.
